Yes, we know it’s a hassle filling out the dreaded doctor’s office paperwork. This is why we offer our paperwork online. This allows you to complete the necessary in the convenience of your own home and it really cuts down on the amount of time needed for your first visit.
Here are the essential forms we use and why we need them.
Health History – CPI
Name, rank, and serial number… Just the standard doctor’s office form.
Automobile Accident
If your health issue is the result of a car accident, we need some additional information.
Release of Records
If you have received chiropractic care elsewhere, or have medical records we will need from another practitioner, please sign this release.
